Every member must—

 

4.1complete and sign an application for membership of the fund on such form and in such manner as the trustees require and return it to the trustees:

 

4.2produce to the trustees a birth certificate or other proof of age acceptable to the trustees [sic]:

 

4.3produce proof of health to the trustees.
