77. Reporting responsibilities of staff

The following incidents must be reported by staff members to the manager of the child and youth care centre within an hour of the discovery or reporting of the incident:

(a)Removal or any attempted removal of a child from the child and youth care centre or programme by anyone who is not permitted to do so;
(b)any situation in which restraint, isolation, or prohibited behaviour
(c)management measures are used;
(d)accident or illness requiring medical attention or hospitalisation;
(e)allegations of physical, psychological, emotional, sexual or verbal abuse;
(f)absence of a child or young person from the centre without permission, or as otherwise agreed within the individual development plan;
(g)interventions by security personnel or the South African Police Service;
(h)the death or injury or a child;
(i)any criminal charge or conviction of a service provider, volunteer or other adult involved with the centre;
(j)any substance abuse by a service provider while on duty or when he or she arrives on duty under the influence of alcohol or drugs or he or she deals in drugs;
(k)any strike by workers at the centre; or
(l)any other unusual circumstances that are likely to affect the safety or well-being of any child at the centre.
